I can't get this to work under my modified OS3.9 ROM's (Boing Bag 1&2 or PPC overclocking issues) so it's up for grabs.

The BVision is clocked from the 68k's oscillator : try with 66 Mhz or 64 Mhz, and your gfx card will works fine :)
Some BVision cards will accept 60MHz oscillator only; even 64MHz is too much.
Retested using 3.1 ROM's and confirmed working! Now unsuitable for my needs so it has to go.

Sorry for commenting here, but since the picture is very nice... I'd suggest that you'll put a Heatsink in the lower right chip below the socket since it gets REALLY hot.
In my Setup I've putted HS+Fan in both this chip and the Permedia and everything runs cool for hours :D
Memory gets a bit hot but not so much to require HS imho. After all there are RAM chips underneath BVision that you can't put HS on due to height
