I've received a CDTV on long-term loan from a neighbour and it's missing the following items:
* CD Caddy - RECEIVED - thanks meega!
* Any form of remote - RECEIVED - thanks Ruud!
* Mouse (unless there's a standard mouse port I haven't found yet...)
If anyone has any of these and will sell them, that'd be great :)
If all else fails, I'll have to buy them new from AmigaKit...
I've got a black CD caddy (nice to have a matching one, eh?) and a CDPD Vol.1 disk kicking around somewhere...

About Workbench CD (not 3.5 or 3.9), I don't think that exists. In terms that you will need Workbench v1.3, as CDTV comes with 1.3 ROMs. The only way to load Workbench is from floppies via an external disk drive.

I still need a mouse and remote though...
Just remember the CDTV have an unique mouse. It is 5-pin mini-DIN (PS/2 style connector but not equal) and use one data line for buttons and movement.
Do you really need one? The remote works as one. Not exactly perfect, but usable.
