Solid lump of copper, lap the face that'll be going onto the cpu, fresh thermal paste and it should do the job. My old itx firewall still works :)

It may not be silent under full load but that is suppossed to be one of the best MITX compatible fans around.
it's about 12cm across but is extremely thin in depth so if case can take it, its the one I'd be getting.
May be worth modding the case internals a little to create a vacuum, similar to what the XBox 360 does.
