This card plugs into the expansion port on the side of the Amiga 500 and has a standard 40 pin IDE connector where you can plug in a standard IDE hard drive or one of the many available CF to IDE adapters. No internal mods required.

I'm personally using this card with a Compact Flash and this CF-to-IDE adapter for a very compact setup with no cables:

This kind of adapter is usually powered from the 4 pin "floppy" connector on the top right. If you're planning to buy the same adapter and you don't want to power it externally I can solder an extra wire to the expansion board so you'll be able to power it directly from the Amiga instead. The wire will go from the card to the floppy connector to provide power.

This is just a suggestion and I'm showing this specific adapter as an example, you should be able to use any other similar adapter, I chose this one because it comes with a female header and plugs straight into the IDE connector with no cables.
I also fixed a small oversight of the original design, originally if you plug in an adapter that doesn't have an "activity" led/signal (or if you leave the card plugged into the Amiga with no hard drive connected at all) the activity led on the board would flash randomly because that signal was effectively left floating. I fixed this with a very small extra resistor on the back of the board, now when the activity signal is not present the red ACT LED stays on and acts as a power led.
