Model MT5634ZPX-PCI - used c. 20 years ago in a dual P3, stored and UNTESTED since and will remain so.
I bought this back then because it's a PCI hardware modem, so there is relatively easy linux compatibility as opposed to "winmodems" and even "linmodems". The manual confirms this insofar as running under linux is straightforward, despite earlier on in the manual saying it's designed for Pentium computers in Windows PnP environments:
