The first picture was taken with Canon EF 70-200 f/4 L USM, the other two with EF 50 f/1.8 II.

I bought the adapter from a guy from Finland called Mika Leinonen, who designed it. This is his website: http://www.students.tut.fi/~leinone3/
I don't know if he still has any of those adapters left in stock, though. Try to contact him.
