I have a Voodoo 3 2000 PCI card to sell to anyone is interested and has a Mediator on the Amiga 
The Voodoo has box, manual and cd drives, so you can also use it on PC with Windows
I have tested and used the card so i can tell you that it works fine
I sell it at 20 euros plus shipping costs ( I live in Italy )
