Phase 5 Digital Products CyberStorm Mk2. I was using this card for the past 4 years without any issues. It was working great with rev6 68060@72MHz. I'll also include original 50MHz cristal.
The board is populated with 128MB of RAM.
There is a small scratch near the KEL connector (visible on bottom picture), but it doesn't affect a card.
The radiators can be easily removed as they're attached using double sided adhesive tape (no glue). The card was not overheating, I just got a pair of spare radiators
