Hi Marc, can confirm my CS-MKIII in my 4000 had no issue installing it with 3.1, 3.14 or 3.2 roms. I would suggest starting a fresh OS install and using only the Blizzard MKIII 060 libs

I have found that Thor's libraries also work fine with 3.2 roms and the CyberStorm MKII.

Well to add my penny's worth if the Guy needs Photos I have mine installed and the only libs I use to have it run is the 060 lib from the Phase5.a1k site it also has the install disks too. I have the Elbox Fast IDE card and the zz9000 the cs MK2 has the full 128MB installed I used the instructions from the Phase5 site years ago and never really thought about it till now LOL. Jumpers are set to INT just checked. http://phase5.a1k.org/files/CyberstormMK2_en.pdf looking at the Manual shows jumpers at INT which Mine are too. I had issues a long long time ago with another 4000D the CPU socket needed to be cleaned..
