Could you make it an actual d-pad? something like the Sega Saturn's would be so much nicer than the default CD32 d-pad (it's terrible imo)

Yes, it's very easy. Simply remove the screws on the back of the controller and remove the PCB and you have the buttons on hand to remove them and replace with the news
