a 4000D, i also have several other dead units i want to sfx like your re 3000 power supply i purchased. that atx adaptod board seems perfect!

For Amiga 4000D you do not need any adapter board (it can be used, but it is not necessary) . Simple cable loom is sufficient. Please check this thread

Firstly I would like to apologise for delay with dispatching orders, I have been away for two weeks.

Mentioned by you PSU seems to be full ATX standard. You need SFX form factor. I used to recommend Corsair SF450, but it's not manufactured any more. Any SFX PSU should be fine, just be aware that some high power SFX may need dummy load on 12V line to work properly.

Check for a Chieftec BFX 450 - it is not very expensive and has also 20A on the 5V rail.
