Summoning Satan from the Mediator 1200 pass-through?

Sitting down here at night, connecting the PSU (https://www.dropbox.com/s/panoxrxji0om17p/IMG_20120530_234311.jpg) to the Mediator 1200 LT4...

The room is dark, the Mediator is connected to the pass-through, to the pass-through the accelerator is attached. There is nothing else but the motherboard, accelerator and pass-through.

A flip of the switch and suddenly a fire instantly builds before my eyes... followed by a mountain of smoke. The not-so-delightful smell of burned, expensive hardware fills the room and I am left with this -> https://www.dropbox.com/s/bjowfz7ie2p66lz/IMG_20120530_232122.jpg

You are now observing the pass-through, nothing else seem to be affected, but I am yet to test the accelerator itself alone, without pass-through and mediator.

Anyhow...
Some soldering should fix the pass-through I hope, but... any help on what may have caused this? How can I prevent it from happening a second time? I can provide any information necessary.
 
You probably have a short circuit somewhere, something maybe touching something else it shouldnt be touching...

Defnitely an over current there my friend.

I'll clean everything up and make sure nothing is touching anything else, then... solder that piece back into place and give it another go. Thanks :)
 
Just take it nice and slow and triple check all clearances..
If you have a meter check you have some resistance between ground and the +5 & +12 supplies before switching on

yep, i would be looking for what actually caused that to happen in the 1st place.

after all the it is a pass through, i don't believe i have seen that before (I know they can stop working if they are moved a lot, as i had to buy a new one)

i know it is impossible to check now, but there is a chance the pass through was not set correctly on the edge connector, misaligning the pins. i know this can happen if you don't seat an accelerator correctly :(

but exactly why I said you had find reason first and not just do the bit you can see
 
I'll be cleaning it up and see if I can get it to work again, no big deal really... A1200 motherboards can be found here and there for cheap, should this one never start again :P

Going to take my time this time around though
