Voodoo Rush, marked ATC 2475, maybe that's the model. The 2D part is Alliance Semiconductor, see pictures.

Great condition. Guessing it's a 6mb card as there are 12 chips around the 3dfx chips (since added screenshot from 3dmark99 which shows 4mb video + 2mb texture which comes to 6mb total).

Tested today given the rig was already set up for testing:
- GLQuake runs with an opengl32.dll at 512x384, the demo that runs when you load the game looked fine, just the usual stuttering numbers on the health bar
- Quake2 kept reverting to software mode so seems to be too much for the card, or I have not set things up correctly
- 3dmark99 max results are shown below

Seems to work for basic Windows 98 gui usage just fine. Quite slow at 3DMark99, as expected.

Obviously very old and potentially fragile hardware so I cannot offer a guarantee other than it was working when tested.
