there is small utility that reads firmware and inform card rev and manufacturer
but I forgot it name and my mac not close to me
sometimes nothing is written on card so case opening won't help
somewhere in internet I think
depends on network card - something was on OS7 disks, something at web, something nowhere
for example, my card is API Engineering and there is neither last driver in Net, nor driver disk
so I use last installation from HD and back up it on my server
check if ethernet driver installed than choose ethertalk
specify IP address for MacTCP (for example 192.168.1.xxx, subnet mask, router address, dns address et voilà)

you placed Village-57 monitor driver
I would not recommend you use CGX4 because of glitches
