Amiga A4000T Audio/Video Module PCB where to find ?

Hello.

I’ve been able to find the A4000T Disk Module, and A4000T Ports Module on PCBWay, but unable to find the A4000T Audio/Video Module — Is anyone aware of where it might be found, so that i can get PCBWay to print a copy of it ?

Also BOM Lists for each of the three modules would be much appreciated (if anyone has them) ?
