Hello 
I have an old 3dfx (possibly dual pcb) card from near the beginning of hardware 3d on PCs. Its on my desk at work and will get a picture next time I am in. That may be a while as my knee is totally knackered atm. Cant even get 5 miles up the road to town without excruciating pain
