I tried to run it with Thor's PCI library too, but with the same result.
I found in PCI-Configuration that my board is ignored on pci initialization. I tried to comment out this lines but it didn't help. Maybe there are some known issues...
I asked on EAB and waiting for answer.

Hi, Can you help me? Why does Prometheus appear disabled for me if I've followed all the steps in the guide?

I have the Amiga 4000d with a BFG9060, and I have installed a 3DLabs Permedia 2V PCI 8 MB card.
Does this card show up when running the “lspci” command in a shell?

Which version of the OpenPCI.library are you using? Use the version OpenPCI.library command to check.
 
lspci doesn't work for me, but an app I found, prm scan, gives me that data.

Okay.

So you are not using a setup with a PPC PCI card?

If not then go for the latest Thors open PCI Library. From here


Use the install script included. This will then install the lspci command into c drawer

Reboot

Try a reinstall of the Icomp p96 software. Choose the permedia2 graphics card. Do not rename the “permedia2” monitor file.

Reboot

Now try lspci to check that the RTG card is seen.

Then try the P96 preferences to setup.

Looking at your lspci output. The RTG card (Permedia2) is not being seen in the list of pci cards. Therefore p96 will not see it either.
You will need to either;

Try the Permedia in another pci slot for it to be recognised
Try the Permedia on its own (remove all other pci cards) to establish you have a working graphics card
Try extra power on the Prometheus Daughterboard with a molex connector

If any of the above don’t work. You may have a faulty permedia2 card.
 
Thank you so much. It was the Permedia2's fault. I installed a Radeon 9250 AGP with an @bfgmatik adapter, and after installing everything, it detected it on the first try. 👏🏼👏🏼
