The Z80 is used on the boot (in order to boot CP/M) then a HALT is issued iirc ..
If you want a HDD interface (IDE64 already exist) for the C128/128D you can use the SD2IEC with CMD partition files.. the problem is some applciations need patching, as they are hardcoded to dev 8 to load datafiles.

What I was thinking is the ability to
1. Load Snapshots (64k and 128K)
2. Save Basic Programs
3. Load Basic Programs
Technically one could write a loader in basic that sits on the HDD and loads the pointed code at a given address to memory and jumps the stack pointer to the begining of the code address and viola' runs the game/application from there. I have done this with the Spectrum version here, and its pretty cool I must say... Although game titles that save continue to point to the FDD or Tape (pending on thier code) - this could be hacked of course...
In the wee small hours last night I was looking into hacking bards-tale on the spectrum for this one
However, if developers want to have at it - to crack games to run from a HDD then all the power to them and I would love to help anyway I can.
Currently the Adapter works at 145KB a second read - combined with plenty of CF or HDD space, I would love to see the demo's made on such cheap hardware.
At the moment the only real thing that holds this up is a 6502 version and ROM code to page in to run the disk access... hardware is very simple enough =)
I would also like to include a folllowing feature -

Hooked upto the NMI as well when I get to re-doing this for the 6502, to be fair I will be including an NMI solder point for a button on the next batch for the Spectrum anyway.
